ASSESSMENT PLANNING

 

Defining and documenting the current skill levels and the likes, dislikes, fears and joys of your special person is a critical step in developing a Future Care Plan.

Life Planning Institute uses extensive skill assessment lists to get an accurate picture of the special person. These lists indicate what a special person likes and dislikes, their capabilities and also whether he/she never tried or would like to learn a particular activity. Categories we review in assessment planning include:

Character Strength Inventory Learning Accommodation Checklist
Education Skill Inventory Learning Style Analysis
Fears Inventory Leisure Activity Inventory
Food Like/Dislike Inventory Physical Activity Inventory
Home Skill Inventory Social Skill Inventory
