WHERE ARE YOU TODAY IN THE PLANNING PROCESS?

Here is a Planning Checklist to help you determine
where you are in the planning process.

Will your Special person have the same
Quality of Life that he/she now enjoys today?

Yes    No   1. Have you defined the supports you believe your special person will need in the future in order to survive in the future?
Yes    No   2. Have you defined the type of housing your child will live in?
(Check the choice that best applies)

a. 

My child will be dependent upon the State for housing and supervision.

b. 

I intend to leave my home as a group home.

c. 

I intend to purchase a condo for my child or to leave funds for that purpose

d. 

I intend that my child with disabilities will live with his or her siblings
Yes    No   3. Do you have a written FUTURE CARE PLAN to let others know what you want in the future?
Yes    No   4. Have you asked someone to serve as an Advocate or Guardian?
Yes    No   5. Do you understand all of the government benefit programs that are available to you?
Yes    No   6. Have you set aside adequate funds so that your Special person will have a comfortable lifestyle?
Yes    No   7. Have you prepared written instructions for the person's final arrangements?
Yes    No   8. Do you or your spouse have current Wills which will exclude your child with a disability?
Yes    No   9. Do you have a Special Needs Trust to manage current and future resources?
Yes    No 10. Have you met with relatives and friends to let them know about your plan?
Yes    No 11. Have you reviewed your Future Care Plan in the last year?
Yes    No 12. Do you feel that you have done everything possible for your Special person's future?

 If you could not answer YES to all of the above, it's probably time
to bring in the Special Needs Team (LIFE PLANNING INSTITUTE)
to begin LIFE PLANNING
