LIFE PLANNING CHECKLIST
1.
Written basic Future Care Plan
2.
Signed and witnessed Last Wills and
Testaments.
3.
Notarized Special Needs Trust agreement.
4.
Received EIN/IRS number, if applicable.
5.
Opened Trust Account at financial
institution.
6.
Discussed plan and given copies of
appropriate documents to proposed Successor Trustees, Guardians, etc.
7.
Changed the beneficiaries excluding disabled
child by name on the following accounts.
All existing life insurance policies (private, employment,
government, military, credit card etc.)
All Pension Plans (employment, private IRAs, Keoghs,
401(k), TSA, etc.)
All other accounts where disabled child may be listed to receive
funds.
8.
Notified relatives and friends that
you established a Special Needs Trust. Please leave any inheritances or gifts to the
Trust.
9.
Notified government caseworkers (Regional Centers,
Social Services, Mental Health, Rehabilitation, etc.) that a trust fund does exist for
supplementary needs.
10.
Placed the following documents in the planning
binder:
Letter of Intent
Medicaid/Medicare applications
Guardianship Court Papers
Copy of Birth Certificate
Copy of Social Security Card
Copy of Medical Card
Brochure from appropriate government agency (Regional Center, Social
Services, Rehabilitation, Mental Health, etc.) with the current case worker's business
card
Trust Account Checkbook
Brochure/Contract with Guardianship Service (NCP, ARC, etc.) and
appropriate business card
Financial Products used to fund the trust (insurance policy,
investment device, etc.)
